Digital Clinical Care: 10 Wk Implementation

We are providing professional consulting services enabling the client to implement a Digital Clinical Care (DCC) experience built on Microsoft Healthcare Cloud. The digital technologies have the power to give software driven recommendation for patient care and enabling non-clinical, MD, pharmacist, and administrator to provide best care pathways. Solution basket leverages the power of Microsoft Cloud for Healthcare and consists of three major components a) Navigators experience and b) MD/ND/Pharmacist and c) Administrator. Following activities are planned: •Customer communication for prioritization of DCC use cases based on customer business objectives, non-functional requirements discussion, and third-party integration approaches finalization, procurement needs •System architecture & design customization, infrastructure topology definition as per the customer need •Implementation of DCC use cases through solution accelerators, and custom code •Integration with third party vendor systems based on specific DCC needs for the customer •Test data preparation, end to end testing of the system •Deployment of the DCC system on Azure cloud infrastructure, publishing of mobile apps Deliverables •DCC scope statement with DCC use cases specific to the business objectives •System architecture using Microsoft Azure & design documents •Care Plan Template & Program Solution Design •Implemented features compiles product & source code •Test plans & scripts used for system verification The following use cases can be solved (but not limited to): •Reduction in operational cost to manage hypertension patients, Quick and efficient development of similar programs for CKD, Lipid panel, Reduction in readmissions The price quoted is indicative and will vary depending upon the type and the number of functionalities to be implemented.
