Description of Services
Life365 is an integrated **Application Integration** partner of Microsoft Cloud for Healthcare providing connected kits with medical devices to patients at home in order to bring in vital sign data into the Microsoft platform and ultimately the enterprise clients clinic backend.
The Life365 has over 15 years of experience in Digital Health, Virtual Care and Remote Patient Monitoring (RPM). The team has built several Remote Patient Monitoring platforms that have been acquired by Fortune 100/500 publicly traded companies. Thru these years, the Life365 team have been thought leaders in the Digital Health / RPM space helping to set standards in FDA regulatory and reimbursement (CPT codes). The team has a deep knowledge of the RPM space and has captured best practices for implementing successful RPM programs.
The Life365 team is available to enterprise clients to help structure their own Remote Patient Monitoring (RPM) program.
Deliverables
- Analysis of the enterprise clients current Microsoft assets (Azure, Power BI, Dynamics 365, etc.), providing a gap analysis of functionality mapped within the Microsoft Cloud for Healthcare platform and the enterprise clients existing platform
- Document the enterprise client’s needs for an RPM program and the gaps that need to be filled.
- Map enterprise clients reimbursement and regulatory strategy to desired program.
- Recommendations on where and how to start using Microsoft Cloud for Healthcare, along with Power BI, Dynamics 365, Azure IoT Hub, Life365 Virtual Care platform, etc.
Scope of Work
Consulting hours are purchased in increments of 10 units.
Complete Statement of Works (SOW) are available upon request for long-term projects.
Additional Services:
(1) Life365 Support (Microsoft Marketplace) - Provide Tier-2 Customer Care and support of Clients RPM program and Patient connected solutions
(2) Life365 API Connect Data (Microsoft Marketplace) - Provide API integration into the Clients clinical backend
(3) Life365 Annual Platform Fee (Microsoft Marketplace) - Annual Fee associated with Clients data being stored in the HIPAA compliant cloud repository
(4) Life365 Integration Services (Microsoft Marketplace) - Integration work that is required to connect the Life365 and Microsoft platform into the Clients clinical backend
(5) Life365 Demo Kits (Microsoft Marketplace) - These are demo Life365 RPM Kits
(6) Life365 RPM Kits - These are the production Life365 RPM Kits
