• Expert coaching support. Highly credentialed and experienced coaches following evidence-based practices provide member-centered guidance and support. Members feel supported and empowered to control their condition.
• Provider-based care. We connect members in poor clinical control to a Teladoc Health provider and care team to access needs, create and execute a care plan, adjust medications, order labs and receive CGM prescriptions in coordination the member’s PCPs and care team. Historically underserved members can finally get the care they deserve.
• Integrated mental health care. Integrated through our mental health suite of solutions. This includes digital programs and activities and licensed coaching all the way through crisis management to address depression, anxiety, stress and other mental health conditions across the acuity spectrum.
